Want to become a new patient?
Please fill out and submit the new patient paperwork using the link below. If you have trouble filling out the paperwork, please use a different browser or device (Chrome seems to work the best). Once we have received and reviewed your new patient paperwork, we will call you to schedule your first appointment. We leave the office on Thursday’s at 1pm and do not return to the office until Monday’s at 8:30am. If you submit your paperwork during that time, we will most likely reach out to you on Monday morning. Typically, we can get new patients in somewhat quickly. If your matter is urgent and you want the soonest available, please let us know that. We are so excited to meet you and we hope that you get miraculous results!
